How do I sign up for a portal?
- Ascension Southeast Michigan & Flint area patients:
To sign up for a doctor’s office portal, call your doctor's office or simply set it up when you are there for a visit.
To sign up for a hospital portal, go to any registration desk at your local hospital. After providing your basic information, you will receive an email with an activation link. Follow the prompts to complete the sign-up process. - Henry Ford Health patients:
To sign up for MyChart, visit https://mychart.hfhs.org/mychart/signup.
In most states, you must be 18 years of age or older to sign up for a patient portal. Some states allow patient portal access at age 13. Please talk with your child's provider to discuss the options for patient portal access for your child in your state.
Can I sign up my child for a patient portal?
Laws vary by state. In some states, you may sign up to get access to your child’s information if they are under age 13. Once they reach 13, a child can give “proxy” access to another adult. Once they turn 18, they need to register for their own account by talking to their healthcare provider. Other states do not allow parents to sign up their child for a portal. Please talk with your child's provider to discuss the options in your state.
Will my information be kept confidential?
Yes. Like your paper-based record, your electronic health record is protected by the Health Insurance Portability and Accountability Act (HIPAA). Your privacy is very important to us and your health information is always kept confidential.
Can I share my password with a family member so that he/she doesn’t need to sign up for proxy access?
This isn’t recommended. Each person should have his or her own account. That way, you will always have the choice in the future to stop sharing your information with another person.
