About Our Community Health Services

Henry Ford Health is committed to improving the health and well-being of our community. We provide services to non-profit organizations such as:

• Health education presentations
• Preventive health screenings
• On-site support at community health fairs

Many services are available at no or reduced cost to help increase access to trusted health information and early detection.

These services are available to worksites, schools, churches, and other community organizations.

Submit a Community Health Request

Scheduling Requirements

To help us plan and coordinate:

• Please submit your request at minimum of 4 weeks before services are needed to help us meet your needs.
• If requesting mammogram services, please allow a minimum of 6 weeks to help us meet your needs.

3. Submit your request for review.

You will receive an email confirmation when your response has been submitted and it will be reviewed by our team.
