Established in 1995, the Center for Health Promotion and Disease Prevention (CHPDP) at Henry Ford Health works to improve health outcomes by advancing prevention, education, and wellness—both across the organization and throughout the communities we serve. Grounded in research, behavioral science, and public health best practices, our work reduces health risks and supports healthier lives for patients, employees, providers, employers, schools, and community partners.

Our Mission

To promote health and prevent disease by:

  • Expanding access to preventive services in clinical and community settings
  • Delivering wellness programs for individuals, employers, and organizational partners
  • Collaborating with health departments, funders, and community-based organizations to reach diverse populations
  • Providing leadership in health promotion, public policy, and community health strategy
  • Advancing research and evaluation to improve how care is delivered

CHPDP is home to Henry Ford Health’s Community Request Portal—a centralized system that connects event organizers and partner organizations with health education, screenings, and outreach services across the enterprise.

Whether you're coordinating a health fair, hosting a presentation, or building a community wellness event, the portal makes it easy to request the support you need.

Preventive Health Services

We support patients, employees, employers, and community partners with access to high-impact preventive care, including:

  • Biometric Screenings: On-site testing for blood pressure, BMI, cholesterol, and glucose—available at worksites, schools, and community events.
  • Medical Nutrition Therapy (MNT): Personalized nutrition counseling provided by Henry Ford registered dietitian nutritionists (RDNs) to support the prevention and management of chronic conditions such as diabetes, high blood pressure, and heart disease.
    • Services are available by physician referral at multiple Henry Ford locations.
    • If your provider is within the Henry Ford Health system, you can self-schedule directly through MyChart.
    • MNT sessions are tailored to individual health goals and include guidance on meal planning, portion control, label reading, and overcoming everyday barriers to healthy eating.

  • Tobacco Cessation Support:
    • Tobacco Treatment Service (TTS): One-on-one phone counseling and medication support for patients and employees ready to quit
    • Freedom from Smoking®: An 8-week group program developed by the American Lung Association, offering peer support and structured steps for quitting

Wellness & Education Programs

We collaborate across the system and with external partners to deliver:

  • Telephonic Health Coaching: Personalized support for lifestyle improvement
  • Health Education Presentations: Expert-led sessions on lifestyle and prevention
  • Worksite Wellness: Custom wellness programming for employers and their teams

Research, Evaluation & Advocacy

CHPDP leads research, education, and advocacy efforts that support:

  • Local and global studies on smoking, health disparities, and vaccine confidence
  • Professional development for healthcare providers and clinical teams
  • Policy advancement and health system improvement, including:
