PACE Southeast Michigan

As people age, they often want to live at home as long as possible. PACE Southeast Michigan helps those who are 55 and older and eligible for nursing home level of care. We bring together all the care they need to enjoy life where they are most comfortable.

When someone enrolls in PACE Southeast Michigan, our team works with them to understand what they need. This care could include help with eating, getting to appointments, making their home safer or getting medical supplies.

PACE has centers around Southeast Michigan so seniors have a place to get medical care, make friends, and enjoy hot meals. We also can take them to medical appointments or bring care to their home. Learn more about our services.

How to qualify for PACE

• Age 55 years or older
• Eligible for nursing home level of care as determined by the State of Michigan
• Living in an area we serve (see below)
• Be able to live safely in the community with our support
