What we offer

Henry Ford Home Health Care’s services are provided by a staff of qualified registered nurses, licensed practical nurses, certified wound, ostomy and continence nurses, physical therapists, occupational therapists, speech-language pathologists, medical social workers, dietitians, and home health aides.

We offer several specialty programs designed to fit the complex needs of our patients. Whether it is high technology care for patients with organ transplants, intravenous therapy or tube feedings, or orthopedic care through the Joint Replacement Care at Home Program, our team is dedicated to providing the highest quality of care.

  • Chronic Care Management

    Managing chronic conditions at home can be a challenge for many patients. If not properly controlled, patients with chronic conditions such as Chronic Heart Failure (CHF) or COPD may make repeated trips to the emergency department or have several avoidable hospital readmissions.

    By combining our skilled nursing and therapy with home monitoring and education, patients can take better control of their health and safely maintain their independence at home.

    Resources for chronic care management include:

  • Joint Replacement

    Our Joint Replacement Care program is designed to help patients safely recover and rehabilitate in the comfort of their own home following total hip or knee replacement surgery.

    We work closely with hospital case managers and physicians to arrange home health care services that will be needed after you leave the hospital. This includes home health nursing and physical therapy. If ordered by the physician, Henry Ford Home Health Care may also provide occupational therapy, home health aide services, medical social work or nutrition consultation.

    Private duty services may be arranged through Henry Ford Extended Care for patients who need extended care for longer period of time on a private pay basis.

  • Specialty infusion and transplant care

    Henry Ford Home Health Care's Specialty Infusion and Transplant Team (SITT) is a highly-skilled team of home health nurses who provide hands-on intravenous (IV) care and mentor other home health nurses on complex IV therapies. We are a national leader in home IV therapy.

    The specialty infusion and transplant team also coordinates the care of solid organ and bone marrow transplant patients, as well as ventricular assist device (VAD) patients.

    SITT supports patients with the following needs:

    • All antibiotic therapies
    • Antifungal infusions
    • Antihemophilic Factor
    • Discontinuation of chemotherapy
    • Dobutamine and milrinone
    • Enzyme replacement therapy including Cinryze, Cerezyme, and others, as ordered
    • Laboratory monitoring for therapeutic drug blood levels
    • Immunosuppression therapy including Atgam, ganciclovir sodium, foscarnet, CytoGam, and other drugs, as ordered
    • IV hydration
    • IVIG, thymoglobulin
    • Maintenance of all vascular access devices
    • Remicade
    • Removal of PICCs and non-cuffed internal jugular (IJ) catheters

  • Remote Patient Monitoring

    Henry Ford at Home’s Remote Patient Monitoring is a personal, in-home monitoring program that combines in home technology with home health services to help patients better manage their chronic conditions. Participation in our Remote Patient Monitoring program may help prevent avoidable visits to the emergency department and hospitalizations while improving your quality of life.

    As part of the program, patients with chronic illnesses such as heart failure, chronic obstructive pulmonary disease (COPD), diabetes or high blood pressure track their vital signs and conditions daily on easy-to-use wireless devices. Patients are also prompted to answer questions about their health and how they are feeling. The information is then transmitted electronically to the telehealth nurse who looks for changes in the patient’s status or results that fall outside normal limits. The telehealth nurse will monitor results and follow up with a phone call or home visit, if needed.

  • Wound, ostomy, and continence nursing

    Our wound, ostomy and continence program provides clinical and consultative services to patients and their caregivers in their homes. This service is provided by a certified wound, ostomy and continence nurse (CWOCN), or a nurse who is extensively trained in specialty wound care.
