Nurse Outreach Coordinators
Nurse outreach coordinators work closely with providers in multiple specialties. Outreach coordinators travel throughout the state and visit practices to provide the latest updates on procedures and therapies available at Henry Ford Health System. In addition, they are able to provide offices with portals exclusive to referring providers in order to refer patients to Henry Ford. The nurse outreach coordinators are a liaison between the referring physician and Henry Ford specialists. Nurse outreach coordinators are able to organize introductory meetings with Henry Ford specialists in locations convenient to the referring provider. They are able to assist with coordination of speakers for Grand Rounds and continuing education events.
Nicole Leon, R.N., B.S.N.
Nicole Leon, RN BSN has been with Henry Ford for over 30 years. Nicole graduated from the Nursing Program at St. Clair College in Canada and later obtained her BSN from the University of Detroit Mercy. Nicole has held many positions throughout her tenure at Henry Ford including several inpatient med/surg units, Advanced Endoscopy and Clinical Trials. In her role in the Clinical Trials Office, Nicole managed oncology patients who were participating in clinical research studies at Henry Ford. Nicole was promoted to Nurse Leader of the Nurse Research Coordinators and Research Assistants prior to joining the RPO in 2017. With her extensive Oncology experience, she has become an asset to the Referring Physician Office where she represents many of the nationally recognized Surgical Oncologists and their multidisciplinary teams. Nicole continues to advocate for patients in this role as she believes that everyone should be aware of the many choices that are available to them while they navigate the challenges of a new or recurrent cancer diagnosis.
Michael Rosenbach, R.N., B.S.N.
Michael Rosenbach, RN, BSN joined the Henry Ford Health System family in 2015 working side by side with talented interventional cardiologists in the Cardiac Catheterization Lab at Henry Ford Hospital Detroit. Mike obtained his nursing degree from Lansing Community College and later earned his BSN from the University of Michigan, Flint. Mike has been a registered nurse for over a decade starting his career in the Lansing metro area working in various nursing capacities. His experience includes Director of Nursing at an assisted living specializing in memory care and home care case management. Mike brings empathy and compassion to work every single day. He specializes in connecting patients and their families with the information and care they need. Mike is currently a RN Outreach Coordinator at Henry Ford Hospital, where he facilitates outreach efforts and helps coordinate the patient care experience for multiple programs including the Structural Heart and Complex High Risk Interventional Programs. Mike supports the tertiary and quaternary subspecialty referrals for Cardiology and Pulmonary.
Maureen A. Sarnacki, R.N.
Maureen A. Sarnacki, RN joined Henry Ford Hospital in 2015 as an RN Outreach Coordinator in the Referring Physician Office. Maureen brought with her a broad range of health care experience as well as marketing and sales experience. Maureen’s clinical background includes over 20 years as a surgical nurse including open heart, cardiothoracic and trauma surgery. Maureen furthered her education and obtained certification as a Registered Nurse Consultant where she provided consulting services to various private practices throughout the state. These experiences allowed Maureen to easily transition into the role of RN Outreach Coordinator. Maureen continues to work with providers throughout the state to ensure the referral process is as seamless as possible. Maureen is committed to ensuring prompt access for referred patients, excellent communication and providing the highest quality of service. Maureen supports the tertiary and quaternary subspecialty referrals for Orthopedics, Urology, non-cancerous Neurosurgery and Neurology.
Christine Siegert,, R.N., B.S.N.
Christine Siegert, RN BSN has been with Henry Ford for over 30 years. Christine holds both a BS in Psychology and a BSN from Wayne State University. Christine has an extensive background in med/surg nursing working on many inpatient units at Henry Ford Hospital. In addition, Christine obtained her Wound Care Certification in 2008 and was on the Wound and Ostomy Team both at Henry Ford Hospital and Henry Ford West Bloomfield Hospital from 2008-2018. Christine joined the Referring Physician Office as an RN Outreach Coordinator in 2020. Christine has enjoyed the variety of positions that she has held throughout her tenure. Christine has enjoyed having the opportunity to work with a diverse group of patients assisting them as they heal and recover. She is looking forward to representing the high quality, ground-breaking care that is offered at Henry Ford and being a liaison between providers and patients. Christine supports the tertiary and quaternary subspecialty referrals for Gastroenterology, Acute Care Surgery/Trauma, Colon and Rectal and Complex Pancreatic Surgeries.
