Case Management

Case Managers are responsible for providing necessary medical services to patients. You, your family or your physician may be contacted by our Case Management staff if there are any questions about your hospitalization or to discuss insurance coverage and your discharge needs.

Your insurance carrier may contact Case Managers during your hospital stay and suggest alternative means of health care, and when appropriate, suggest discharge from the hospital. The Case Managers will inform your physician if your insurance carrier makes any such directive.

Case Managers are assigned to each unit to help you with discharge planning. Discharge planning is a multi-disciplinary hospital-wide process initiated at the time of admission. Case Mangers will identify patients requiring complex discharge planning, such as coordination of community resources, long-term care (sub-acute, nursing home, etc.), facilitating referral to home care agencies, hospice, or contacting durable medical equipment companies.

Case Managers also provide crisis intervention and counseling for patients and their families with a focus on post-hospital planning needs.
