  • How do I collect my samples?

    The following instructions are for breast milk and formula. You may collect one or the other. You do not need to collect both. Please attach the correct “Breast Milk” or “Formula” label onto the collection container. 

    Breast Milk Collection Instructions

    We appreciate your participation in the REACH study. We are asking you to provide a sample of your breast milk following the instructions below. Breastmilk contains lots of nutrients and other good things for your baby, and researchers would like to measure those things and find out how they help your baby grow and develop! This information could help to advance mom and baby health in the future. If you have any questions or concerns, please call us at (313) 874-3151. Someone on our study team would be happy to answer any questions or walk you through the process. 

    What’s in your kit:  

    • (1) Breast pump (First collection only) 
    • (1) Collection container
    • (1) Biohazard bag
    • (1) Sample collection sheet
    • (1) Ice pack
    • (1) Breast milk or formula container

    Tips:

    • Please collect your breast milk in the morning on the day of your scheduled visit. This does not have to be your first expression of the morning.  
    • Make sure you hold the pump rather than the bottle. Holding the pump by the bottle can lead to blockage of your milk ducts.  
    • If you experience any trouble or pain while expressing, please call your doctor for help. 
    • We are only looking for approximately ½ oz of breast milk. That is less than a tablespoon. 

    Instructions:

    1. Place the gel ice pack provided in the freezer the night before your scheduled appointment.  
    2. Assemble the pump according to the instructions provided with it.  
    3. Remove the collection container from the sample kit and set it down on a counter or table. 
    4. Inspect the pump for any cracks. If there are any issues with the pump, feel free to reach out to the study team. 
    5. Center the breast shield over your nipple. 
    6. Begin pumping.  
    7. As soon as you have finished pumping, remove the bottle from the pump. 
    8. Pick up the collection container from the counter and unscrew the lid. 
    9. Transfer ½ oz of your breast milk from bottle into the collection container provided to you in the sample kit.  
    10. Place the lid securely on the collection container provided. 
    11. Mark the date and time of your breast milk sample collection on your sample collection sheet. Check off that the breast milk sample is complete!  
    12. Place the collection container in the biohazard bag provided in the kit. Then place the sample in the sample bag provided to you.* 
    13. Place the sample bag into the refrigerator until you leave for your clinic appointment.  
    14. Add the frozen ice pack in your sample bag when you leave for your appointment to keep your sample cool.  

    *The samples you are collecting can all go in the sample bag together! It is also okay for all the samples to be refrigerated.

    Formula Collection Instructions 

    Thank you for your continued participation in the REACH study. Formula is an important specimen for our study. Infant formula contains just the right amount of nutrients and other good things that your baby needs, and researchers would like to measure those things to find out how they interact with other stuff in your environment to help your baby grow and develop! This is the sort of information that helps to advance women’s and children’s health for years to come. You can collect the sample following our instructions below. If you have any questions or concerns, please call us at (313) 874-3151. A member of our study team will be happy to walk you through the process or answer any questions you might have.

    What’s in your kit:

    • (1) Collection container 
    • (1) Biohazard Bag 
    • (2) Sample collection sheet

    Tips:

    • Please make up/collect your formula in the morning on the day of your scheduled clinic visit. 
    • Please collect formula as soon as you are finished preparing it, before baby has the bottle.  

    Instructions:

    1. Place the ice pack provided in the freezer the night before your scheduled appointment.  
    2. Remove the tube from the sample kit and set it down on a counter or table. 
    3. Right after you make a bottle of formula for your child, transfer ½ oz of prepared formula into the collection container provided to you. Please do this before the bottle is given to your baby. 
    4. Tightly close the lid of the filled collection container and place it in the biohazard bag provided. Then place the sample in the sample bag. * 
    5. Mark the date and time of your formula sample collection on your sample collection sheet. Check off that the formula sample is completed!  
    6. Place the sample bag into the refrigerator until you leave for your clinic appointment.  
    7. Place the frozen ice pack in your sample bag when you leave for your appointment.  
    8. At your next visit, please bring this formula sample with you.

     *The samples you are collecting can all go in the sample bag together! It is also okay for all the samples to be refrigerated. 


    Urine Diaper Collection Instructions for Baby

    Thank you again for your participation in our REACH study. Our study values specimens as a key part to research. We are asking that you collect your baby’s urine by following the instructions below. This sample can help us understand what your baby is exposed to in their environment, which can help advance the future of baby health! If you have any questions or concerns, please call us at (313) 874-3151. A member of our study team will be happy to walk you through the process or answer any questions you might have. 

    What’s in your kit:

    • (1) Pair of gloves 
    • (1) Pack of diapers 
    • (4) Cotton pads 
    • (1) Urine collection container 
    • (1) Plastic forceps 
    • (1) Biohazard bag 
    • (1) Sample collection sheet 
    • (1) Ice pack 

    Tips:

    • The best time to collect this sample is the day before your child’s visit. 
    • If your baby does not regularly urinate during the day, try for an overnight urine collection the night before their scheduled visit. 
    • If your baby does stool in the diaper, simply discard everything and try again in the morning.   

    Instructions:

    1. Place the gel ice pack in the freezer for at least 12 hours before the visit (this gel ice pack will be used for transporting your samples). 
    2. Prepare the provided diaper as you would normally to place on your baby. 
    3. Place 2 cotton pads into the diaper where the diaper is likely to be the wettest. For boys, the pads should go more in the front of the diaper. For girls, the pads should go more in the middle of the diaper.  
    4. Wait for your child to urinate, or until the next morning if you are putting the diaper on overnight.  
    5. Remove the diaper, set it aside, and finish changing your child.  
    6. Take the top off the cup while avoiding touching the inside of the container or lid.  
    7. You can use the tweezers provided to transfer the wet cotton pads into the urine cup or your gloved hands is fine too. 
    8. Make sure the cotton pads are completely saturated with urine. Then, use the tweezers provided to you to transfer the wet cotton pads into the urine cup. 
    9. Remove the gloves and wash your hands. 
    10. Place the urine cup into the biohazard bag provided in the kit. Place the bagged sample in your sample bag.* 
    11. Wash your hands. 
    12. Place the sample bag in the fridge or place the frozen ice pack in your sample bag to keep the sample cool until your appointment.  
    13. Mark the date and time of your child’s sample collection on your sample collection sheet.  
    14. Remember to bring the sample with you to your child’s next scheduled visit. Use the sample bag and icepacks provided to transport all samples.  

    *The samples you are collecting can all go in the sample bag together! It is also okay for all the samples to be refrigerated.  


    Urine Potty Trained Collection Instructions for Baby

    Thank you again for your participation in our REACH study. Our study values specimens as a key part to research. We are asking that you collect your baby’s urine by following the instructions below. This sample can help us understand what your baby is exposed to in their environment, which can help advance the future of baby health! If you have any questions or concerns, please call us at (313) 874-3151. A member of our study team will be happy to walk you through the process or answer any questions you might have.

    What’s in your kit:  

    • (1) Urine cup 
    • (1) Biohazard bag 
    • (1) Sample collection sheet 
    • (1) Ice pack 

    Tips

    • Collect the urine sample either on the same day as your child’s visit or the night before and place in the fridge. 

    Instructions

    • Place gel ice pack in freezer at least 12 hours prior to your scheduled visit (this gel ice pack will be used for transporting your samples). 
    • Remove the lid from the urine cup. Make sure the inside of the container and lid remain untouched. 
    • Have your child sit on the toilet. Instruct them to wait to pee until you say. This step is pictured below.
    • Holding the cup, reach into the toilet under your child.  
    • Tell them it is okay to start going! Once they have started to pee, place the cup directly under the stream.  
    • When they have finished, place the lid securely on the cup. If necessary, wipe the outside of the cup with a tissue and discard the tissue. 
    • Wash hands. 
    • Place the urine cup into the biohazard bag provided in the kit.  
    • Place the bagged sample in your sample bag and place the sample bag in the fridge or the frozen ice pack in the bag to keep samples cool.* 
    • Mark the date and time of your child’s sample collection on your sample collection sheet.  
    • Remember to bring the sample with you to your scheduled visit. Use the sample bag and ice packs provided to transport all samples.  

    *The samples you are collecting can all go in the sample bag together! It is also okay for all the samples to be refrigerated. 


    Stool - Diaper Collection – Collection Instruction Sheet

    Thank you for your participation in REACH! An important part of our study is collecting specimens that will help us learn more about how the environment affects our bodies. This information could help improve the health of moms and babies for years to come! We are asking you to provide a stool sample from your child using the instructions below. If you have any questions, please call us at (313) 874-6433. A member of our study team will be happy to walk you through the process!

    What’s in your kit: 

    • (1) Pair of gloves
    • (1) Diaper
    • (1) Stool tube with swab
    • (2) Biohazard bags
    • (1) Sample collection sheet

    Important:

    • Gel ice packs should be placed in the freezer for 24 hours or longer prior to stool collection.
    • Stool will be collected from your child’s diaper(s)

    Instructions:

    Throughout the 48 hours before your scheduled appointment, save as many of baby’s stooled diapers as possible in the refrigerator.

    A. For ONE of these diapers, please complete the following:

    1. When ready to collect stool specimen, place a new diaper on the child. No creams or powders should be used. Do not open the collection tubes yet.
    2. Periodically check the diaper for stool.
    3. Once your child has a bowel movement, take out the gloves and tubes, with caps still in place.
    4. Put on the gloves if you would like and remove the diaper and set aside.
    5. If there is more than just liquid stool in your baby’s diaper, finish changing your child.
    6. Uncap the collection tube containing liquid.
    7. Unwrap the swab and hold it from the top. Please do not let the bottom of the swab touch anything but your baby’s stool.
    8. Run the swab through your baby’s stool while rotating, to get an even coverage of stool over the entire swab.
    9. If the stool has all been absorbed by the diaper, try running the swab along parts of your baby’s bottom to cover the swab as much as possible.
    10. Place the stool-covered swab into the tube. Break off the top half of the swab so that it fits inside!
    11. Tightly recap the tube.
    12. Shake the tube vigorously for 30 seconds to mix the stool specimen with the liquid.
    13. Place the swab tube in a biohazard bag.
    14. Wrap the diaper up as you normally do.
    15. Place the diaper in another biohazard bag.
    16. Remove and discard gloves and thoroughly wash your hands.
    17. Place the biohazard bag into your cooler bag. Refrigerate.
    18. Mark the date and time of your child’s sample collection on your sample collection sheet. Check off that the stooled diaper is completed!
    19. Remember to bring the sample with you to the appointment. Use the cooler bag and icepacks provided to transport all samples. 

    B. For ALL OTHER diapers you were able to save, please complete the following: 

    1. Wrap the diaper up as you normally do.
    2. Place the diaper in another biohazard bag provided.
    3. Remove and discard gloves and thoroughly wash your hands.
    4. Place the biohazard bag into your cooler bag.
    5. Refrigerate until the above swab has been completed and you are ready to head to your appointment. 
    6. Mark the date and time of your child’s sample collection on your sample collection sheet. Check off that the stooled diaper is completed! 

    Remember to bring the sample with you to the appointment!


    Toenail and Fingernail Collection Instructions for Baby

    Thank you for your continued participation in REACH. Our study is all about collecting specimens, which then can help us learn about how the environment affects our bodies. We are asking you to provide a toenail sample from your baby using the instructions below. The toenail samples can give us clues about what metals a baby might be exposed to! This information will help improve the health of babies for years to come. If you have any questions or concerns, please call our study phone at (313) 874-3151, and a member of our study team will be happy to walk you through the process or answer any questions. 

    What’s in your kit:  

    • Small white envelope 
    • Biohazard bag
    • Sample collection sheet

    Please collect your baby’s toenails and fingernails every time you are clipping them, from when you receive the kit until it’s time to bring it in. All the nails will go into the same small envelope.

    Tips: 

    Sometimes nail clippings like to fly across the room. To prevent you from losing too many, here are some ideas: 

    • One strategy is to clip your baby’s nails after they’ve had a bath. Their nails will be softer, and some people find that this helps the nails stick to the clippers. 
    • Try clipping your child’s nails into an empty grocery bag or trash bag. Open the bag, place your child’s foot partially inside the bag, and then trim! Simply collect the toenails from the bottom when completed and put them into the collection envelope. 
    • Clip nails on top of a dark surface such as a towel. Then it will be easier to see them once they are clipped off. You can pick up these fallen nail clippings and put them into the collection envelope. 

    Instructions: 

    • The first time you do this, record the start date of clippings on the sample collection sheet. 
    • Using either the small clippers provided to you or your own clippers, trim your child’s toenails as you normally would. Try to collect from all 10 toes. 
    • Next, trim their fingernails. 
    • Place as many of the nail clippings as you can into the collection envelope.  
    • Repeat steps 2 and 3 each time you clip your baby’s nails. 
    • Store the envelope in a safe place in between clippings.  
    • Before your scheduled visit, seal the envelope and place it into the biohazard bag.  
    • Place the biohazard bag in the sample bag provided to you.* 
    • Record the end date of clippings on the sample collection sheet. 
    • Check off that the toenail/fingernail sample is completed!  
    • At your next scheduled visit, please bring this nail sample with you.

    Hair Collection Instructions for Baby

    We appreciate your continued participation in our REACH study. Collecting specimens is important for our study, so that we can see how environment affects the human body. We are asking you to provide a hair sample from your baby following the instructions listed below. Your baby’s hair sample could give us clues about their hormone levels! This is information that will help improve baby health for years to come. If you have any questions or concerns, please call us at (313) 874-3151. A member of our study team will be happy to walk you through the process or answer any questions you might have. 

    What’s in your kit:  

    • Small white envelope 
    • Biohazard bag
    • Sample collection sheet 

    Tips: 

    • Your child’s hair does not need to be freshly washed for collection. We want this to be as simple and convenient as possible. 
    • Make sure you only use hair collected directly from your child’s head. Do not use hair that falls out naturally or hair left on a brush or comb.  
    • We only need 5 strands of hair. This helps us get accurate information, but it’s also barely noticeable that it was cut. 

    Instructions: 

    • Choose a small section of hair to cut (wherever is easiest for you). 
    • Cut 5 strands of hair as close to your child’s scalp as possible. Make sure you use scissors instead of plucking, ripping, or tearing strands of hair.  
    • Place the strands of hair into the small white envelope provided to you.  
    • Seal the envelope and place it in the small biohazard bag.  
    • Place the biohazard bag in the sample bag provided to you.* 
    • Mark the date and time of your child’s hair sample collection on their sample collection sheet. Check off that the hair sample is completed!  
    • At your child’s next scheduled visit, please bring this hair sample with you.  

    *The samples you are collecting can all go in the sample bag together! It is also okay for all the samples to be refrigerated.  


    Nasal Swab Instructions for Baby

    Thank you for your participation in REACH! An important part of our study is collecting specimens that will help us learn more about how the environment affects our bodies. This information could help improve the health of moms and babies for years to come! We are asking you to provide a nasal swab sample from your child using the instructions below. If you have any questions, please call us at (313) 874-3151. A member of our study team will be happy to walk you through the process! 

    Reagents and/or Supplies (per visit)  

    • 1 sterile Copan Pediatric FLOQ-Swab 
    • 1 small clear tube with 1ml Zymo Shield Reagent
    • 1 Biohazard bag 

    Instructions:  

    1. Wash hands
    2. Place small clear tube on a flat surface.
    3. Remove cap from tube and place on the counter.
    4. a. If you have partner available to support: Have your child lie down and tilt their head back to position your child for nasal swab collection. A partner may need to hold the child’s head in place to minimize movement that could cause discomfort or injury. Additional holding methods may be utilized for young children, as needed. 
      b. If you do not have a partner available to support: Tilt child’s head back gently, with one hand to steady chin.  
    5. Unwrap the white-capped tube with the swab. Remove the swab from its packaging while being careful not to touch the swab to any surface or body part other than the inside of your child’s nose. Discard the white-capped tube the swab came in.
    6. Insert swab into one of your child’s nasal passages until a slight resistance is met keeping the swab near the septum. 

    7. Rotate swab a few times then hold swab in place for a couple seconds to ensure maximum absorption. 
       
    8. Remove the swab from the nostril. 
    9. Place the swab into the uncapped small clear tube.   
    10. Snap off the excess shaft by bending the swab shaft once or twice at the pink dot Cap the tube tightly with the blue cap.   
    11. Shake tube with swab in it vigorously for 5 seconds. 
    12. Place in a biohazard bag and give to study personnel. 

    Vacuum Dust Collection Instructions

    *Not every mom will need to collect dust. This is only if you answered YES to the survey question asking if you were willing to provide it!

    We appreciate your participation in the REACH study. We are asking that you collect vacuum dust samples following the instructions below. This sample can show us information about the microbiome of the home environment. Our interactions with our home environment can impact our health! This information will be used to advance mom and baby health in the future. If you have any questions or concerns, call us at (313) 874-3151 and a study staff member will be happy to talk you through it.

    What’s in your kit:

    • (1) Scoop spoon
    • (1) Collection container
    • (1) Biohazard bag
    • (1) Sample collection sheet
    • (1) Pair of gloves

    Tips:

    • To avoid a mess, place the collection container in a plastic trash bag to collect anything that spills out of the cup. 
    • We have included optional gloves for you to wear if you don’t want the vacuum bag contents touching your hands.

    Instructions: 

    1. Open the collection container.
    2. Open the dust collecting container of your vacuum cleaner and put on the gloves if you would like.
    3. Carefully scoop a small amount of vacuum dust into your collection container or pour a small amount of dust from your vacuum into the collection container.  See tip above  -  it is easier to do this over a trash bag or can.
    4. Close the collection container and place it into the biohazard bag provided to you.
    5. Write the date and time of your vacuum dust collection on the sample collection sheet. 
    6. Check off that your vacuum dust sample is completed.
    7. Please bring this dust sample with you to your next scheduled visit!

    Keep in mind: 

    • You may be used to getting your survey links emailed to you, however Diet ID works a bit differently. You will be emailed your log in information and will then be able to click on the website link to log in!  
    • You will complete this in the 1st trimester, 2nd trimester, and 3rd trimester and again at 1 month postpartum  
    • You will receive an infographic as a reminder to complete this at all time points 
    • As a thank you for completing your Diet ID questions, we want to offer you the opportunity to schedule a 30-minute phone call with our dietitian to have your results explained to you!

    Shipping Instructions:

    1. Samples need to be shipped back to the REACH study on a Monday, Tuesday, or Wednesday.
    2. Place the biohazard bag into the shipping box along with any other collected samples and place the return label on the box.
    3. On the day you ship samples back, please contact the REACH team to let them know. You can text (313) 207-1483 or email REACH@hfhs.org.
    4. Take to your nearest FedEx location and drop box off.

    Who do I give my samples to? 

    Let the front desk receptionists know that you are a participant in the REACH study and they will take your samples for you. 

    What if I forget my samples? 

    • Scenario 1: If you forget your samples and have already completed them, you may bring them back to the clinic within 24 hours of collection. 
    • Scenario 2: If you forget your samples early within your appointment timeframe and you have not yet collected them, you can bring them to your next appointment. 
    • Scenario 3: If you forget your samples late within your appointment timeframe and you have not yet collected them, you can mail them in. Please contact us.

    Will I still be paid if I forgot my samples? 

    You will be paid for each sample you provide and can earn up to a certain amount for full participation which can be seen in the table above.

    How do I use the ClinCard? 

    • In-Store Purchases (by selecting either the “Credit” or “Debit” option) 
      • Using the “Debit” option required a PIN (see “How to set Pin” below)

    • ATM to get cash (requires PIN) 
    • Cash advance at a bank: 
      • You must know your available balance and inform the teller of the amount you would like to withdraw. Tellers are unable to check your balance (see “How to check your balance” below). 
      • Present the teller with your signed ClinCard and a valid government-issued photo ID 
      • The ClinCard is accepted at all MasterCard member banks (look for a MC logo on the bank/window entrance)

Please sign the back of the card and then you can immediately begin using it by selecting “credit” option in stores or for online purchases. If you would like to use the card at an ATM or get cash-back with the debit function, you will need to create a PIN. 

    How do I set a PIN? 

    Call Customer Service at 1-866-952-3795 and follow the prompts through the interactive voice response (IVR). 

    How do I check my available balance? 

    • Call Customer Service 1-866-952-3795 and follow the prompts through the IVR 
    • Login to MyClinCard.com 
    • Balance Inquiry at ATM (requires PIN) 
    • Inquire at your next office visit 

    Are there any fees when using my ClinCard? 

    There are no fees* for: 

    • Making online or in-store purchases 
    • Cashing out the card by presenting it to a teller at any major bank 
    • Calling the automated system for balance inquiries 
    • Calling the Customer Service number and speaking to a live agent 
    • Addition of funds to the card (Note: Funds can only be added to the card by the site) 

    The following activities will incur a fee* to the balance on your ClinCard: 

    • Not using the card or having funds added for 12 months. Please contact 1-866-952-3795 if you receive an inactivity fee.  
      • We have scheduled our payments to you to keep your account active.  If you complete all the sample collections/surveys you will not incur this fee during the study. 
    • ATM withdrawals (fees vary based on location) 
    • Requesting a paper statement. Instead, you can always check your available balance online or by calling Customer Service. 
    • Requesting a replacement card through Customer Service. Instead, your study site can replace your card at no charge.

    What if the amount of the purchase is for more than the balance on my ClinCard? 

    If the payment due is for more than the available balance on your card, be sure to inform the cashier of the designated amount you would like to use. Otherwise, if the card is authorized for an amount higher than the available balance, it will be declined.  

    What should I do if my ClinCard is lost, stolen or damaged? 

    If your card is lost/damaged, notify your point of contact at the study site (email: REACH@hfhs.org, phone number: (313) 874-3151), and they will be able to replace it for you at your next appointment at no charge. This will deactivate your old card, transferring any remaining balance to your new card. If you prefer to contact customer service, there is a $7.00 fee for replacement, and it will take 7-10 days to receive by mail. 

    If your card is stolen, call Customer Service 1-866-952-3795. Customer Service will mark the card “stolen” and will assist you in contacting MasterCard to open a case. 

    Can I use my ClinCard at a restaurant? 

    Yes. However, please take your ClinCard inside to the cashier and ask them to run the card for a specific amount. Otherwise, if you use the card at the pump, the gas station will preauthorize your card for up to $100 or more. While the preauthorization is not a charge made to the account, as long as it is in place, it factors into the calculation of the available balance. It can then take several days for the preauthorization amount to be removed.
