Biometric Health Screen

You have two options:

Attend an onsite screening at MACI between November 16 and December 11, 2026. Fasting for 12 hours prior is required. The link to schedule an appointment can be found on the MACI homepage: Michauto.com as well as in an enrollment email from It’s Your Life (IYL) if you participated in the program in 2026.

Or

Have your screening done at your physician's office and return the Physician's Results Form to IYL by fax or place it in the drop box at MACI Corporate Services. We will accept blood work, blood pressure, weight and height values from January 1, 2025, through December 11, 2026. The deadline to return this form is December 11, 2026. A Physician’s Results Form can be picked up at MACI Corporate Services and is attached in the enrollment email from IYL if you participated in the program in 2026.
