Tapan is currently a part of ambulatory Pharmacy team at Henry Ford Wyandotte Hospital-Outpatient Pharmacy. Tapan Joined Henry Ford Health in 2017. Prior to Henry Ford, Tapan has worked in various community Pharmacy settings for five years. He is also specialized in delivering medication therapy management and vaccinations at outpatient settings. He is currently working on a medication reconciliation pilot project to minimize medication errors and therapeutic duplication at point of discharge to reduce hospital readmissions. Tapan firmly believes that changes in our nation’s health care field are inevitable and how we adopt to it makes all the difference. He is interested in digital health care to make the discharge workflow easier and efficient for patients and providers. Outside of work, Tapan plays a role of husband and a father of one year old at home. Tapan enjoys music and spending time with family.
