The HFPN combines the resources and diversity of Henry Ford Health providers and hundreds of independent providers to create a united physician network that places its focus on offering all of its patients the most appropriate care in an expedited timeframe by highly regarded physicians.

The HFPN has built a robust PRM team that provides all points of data reporting needed to advance the value-based work of the Network. The PRM team continually gathers and contextualizes data to help evaluate overall success on a Network level, while and also to drilling down to the practice level.
