What is a Direct-to Employer Plan?

A direct-to-employer (D2E) plan connects a healthcare organization and its providers directly with an employer’s benefits/HR team to co-create a medical plan that supports employees, their dependents and the company’s financial obligation.

HFPN Providers and Facilities

The HFPN is a clinically integrated network of more than 2,800 providers, including the Henry Ford Medical Group, independent specialists and primary care physicians across southeast Michigan.

Employees gain in-network access to the entire Henry Ford Health system—hospitals, medical centers, urgent care, same-day clinics, labs, radiology and nationally recognized specialty care in areas such as cancer, cardiovascular health, neurology, orthopedics, urology and transplant.

Concierge Service

Our D2E plan includes dedicated concierge support. This service helps employees select providers, schedule appointments, answer coverage questions and navigate their benefits with ease.
