Value-Based Contracts
HFPN has several value-based contracts that providers may be eligible to participate in, which aim to align incentives, improve outcomes and lower costs:
- General Motors ConnectedCare
A direct contract between General Motors and Henry Ford Health. Members receive access to the full Henry Ford Health system, including HFPN providers, Population Health Management programs and enhanced member support. Enrollees can complete their annual preventive visit virtually. - Henry Ford Health Employee Plans
HFHS Advantage Tiered Access and CDHP Comprehensive HFHS Preferred Network plans are two plan options available to Henry Ford Health employees that reward care delivered by HFPN providers. - HAP Pivotal
A commercial product for employers with 100 or more employees. - HF Choice
An exchange-based product for individuals. - Mosaic ACO
An Enhanced Medicare Shared Savings Program (MSSP) contract serving nearly 50,000 Medicare beneficiaries. Available to providers within HFPN and Covenant HealthCare Partners.

Virtual Preventive Visits
GM ConnectedCare members may complete their annual preventive visit virtually. This makes scheduling easier and ensures they receive their HSA credit from GM before the end of the calendar year.

Epic Through Community Connect
Epic helps physicians deliver safer, more coordinated and cost-effective care. By using a single system across the network, providers can share clinical data instantly, avoid redundant testing and improve continuity of care for patients.
Through the Henry Ford Health Community Connect program, independent physicians can access Epic at a discounted rate. The program provides full support — from workflow assessments and customized build to training, go-live and ongoing optimization. Practices also receive post-implementation improvement sessions, monthly onsite support and twice-yearly user forums.
Each deployment includes clinical documentation, registration, scheduling and the option to add integrated professional billing.

Referring Physician Office
All HFPN members will have the resources of an extensive Referring Physician Office to help guide patients to the care provider that best serves their needs.
