Henry Ford Health’s Mosaic Accountable Care Organization Earns $27.2 Million in Shared Savings

DETROIT – Henry Ford Health, a longtime leader in population health management and value-based care, announced its Mosaic Accountable Care Organization (ACO) achieved $27.2 million in shared savings for calendar year 2024, with $19.9 million retained for reinvestment in in high-value care initiatives. The performance represents a 4.5% savings rate, surpassing previous years’ results.

“This achievement demonstrates the strength of physician-led collaboration and the value of preventive, patient-centered care,” said Dr. Jerry Finkel, President and CEO of the Mosaic ACO and Henry Ford Health’s Chief Primary Health Officer. “By focusing on managing chronic conditions and supporting patients proactively, our providers are helping people live healthier lives while reducing unnecessary costs.”

Mosaic ACO participates in the Centers for Medicare & Medicaid Services Medicare Shared Savings Program. The organization unites providers from Covenant HealthCare Partners and Henry Ford Physician Network — including Mosaic Clinically Integrated Network and independent providers — to deliver coordinated, high-value care that improves quality, lowers costs, and enhances outcomes for patients and communities.

An ACO is a group of doctors, hospitals, and other health providers that voluntarily come together to deliver coordinated, high-quality care. The Centers for Medicare & Medicaid Services sets a spending benchmark for an ACO’s Medicare patients. If the ACO meets quality requirements and reduces total spending below that benchmark, the savings are shared — with a portion returned to the ACO and a portion retained by CMS.

In 2024, Mosaic ACO placed particular emphasis on improving care for patients with chronic illnesses such as congestive heart failure, COPD and diabetes. Data-driven strategies and care coordination helped providers deliver more effective treatment, reduce hospitalizations, and support patients in managing their conditions at home. These outcomes represent improved health for more than 45,000 Medicare beneficiaries, underscoring providers’ commitment to quality, accountability and partnership.
